Trump announces $90 Medicare payment for more than 20 million seniors
US President Donald Trump has announced a one-time $90 payment for more than 20 million people enrolled in Medicare, with the money intended to help cover the cost of their Medicare Part B premiums.
According to the White House, the payments will begin in early October. Most eligible beneficiaries will receive the money through direct deposit, while those without direct deposit will receive a check by mail.
The payments will be financed through the Medicare Improvement Fund, a federal fund created in 2008 to support improvements to the Medicare fee-for-service program. Congress has allocated $2 billion to the fund, which the Trump administration says had not previously been used to make direct payments to beneficiaries.
The White House said most Medicare Part B enrollees will qualify for the payment. Beneficiaries whose Part B premiums are already covered by Medicaid, as well as those who pay an income-related monthly adjustment amount, are excluded from the payment program.
The payment comes as Medicare costs remain a major issue for older Americans. The standard monthly Part B premium is $202.90 in 2026, up from $185 in 2025, while the annual Part B deductible increased to $283.
Trump has presented the measure as part of his administration’s efforts to reduce healthcare costs for seniors. His announcement also comes alongside other financial measures, including separate $500 refunds for nearly one million Americans linked to Affordable Care Act marketplace fees.
The $90 payments are being announced just weeks before the November 2026 congressional midterm elections. The timing has prompted political debate over the administration’s broader use of direct financial relief, although the payment itself is formally tied to the Medicare Improvement Fund and Medicare Part B costs.
The administration has also promoted other proposals involving direct payments to Americans, including Trump’s previously discussed plan for $5,000 payments, which would depend on future congressional action. The $90 Medicare payment, however, is a separate measure and is already scheduled for distribution in October.
